Concussion Testing Dates for FALL Athletes that are current students

Concussion testing for current Hammond HS students will take June 9th, 10th, and 11th at 2:30 PM in the Back Media Computer Lab.  We will also offer Concussion Testing for ALL Fall athletes 8/4, 8/5, and 8/10 at 9:00AM in the Back Media Computer Lab. PLEASE bring BOTH forms signed by the athlete and parent/guardian.  The two signed forms are REQUIRED for testing.  Links to the forms are below under REQUIRED PAPERWORK FOR HCPSS SPORTS PARTICIPATION.  Hard copies of both forms are also available on the Hammond Athletics bulletin board outside of the gymnasium.

Concussion Testing for RISING Hammond 9th Graders

Concussion testing for rising 9th grade fall athletes who will attend Hammond High will take place at each of our feeder middle schools.  Dates are coming soon!  Rising 9th graders are also welcome to come to the 8/4, 8/5, and 8/10 sessions at Hammond HS at 9:00 AM in the Back Media Computer Lab.

Save the date: Fall Sports Parent Meeting and Paper Work Turn in 

  • When: August 6, 2015 at 6:30 p.m.
  • Where:  Hammon HS Cafeteria
  • Who: ALL Fall Athletes and a parent should attend.
  • Note:  Please bring with you ALL required paperwork for sports participation (see links below).

REQUIRED PAPERWORK FOR HCPSS SPORTS PARTICIPATION:

  • Copy of Birth Certificate – This is only needed 1 time and is kept on file for all 4 years.
  • Copy of BGE Bill, Lease, Cable Bill, or Bill that is tied to your home – This is your proof of residency in Hammond High School’s district.
    • One copy of your proof of residency is required each season (NEW HCPSS POLICY FOR 2013-2014).
  • Physical Form – This is required each year before the student-athlete participates in interscholastic athletics.
  • Concussion Release Form – Each student-athlete is required to take a base line concussion test every other year. The test is offered at HHS and there is no charge for the testing.
  • Concussion Information Sheet – The concussion release and the information sheet both must be signed and brought to the test by the student-athlete
  • The following form is required for each sport season the student-athlete is participating in.
    • Parent Permission Form – Please make sure that it is signed by both the student-athlete and a guardian, insurance information is included, and the address of the student-athlete is written.
  • Sudden Cardiac Arrest Information Sheet - Please make sure that it is signed by both the student-athlete and a guardian.
